| | Since the year 2000, PSMG has offered the combined technology of TriPath Imaging: the FocalPoint (tm)slide profiler and SurePath (tm)liquid based Pap Testing. PSMG will supply collection materials at no charge from the selection below.
Upon request, PSMG will provide prefilled, liquid based SurePath Pap smear vials and collections devices. MSDS (Material Data Safety Sheets) are available.
Ancillary tests may be added to the SurePath Pap smear without altering collection process. These tests may be requested at the time of specimen submission or later. Tests include: Gonorrhoeae/Chlamydia, HPV, and HSV I/II. Please see “Ancillary Testing” below. | |

| | SurePath Pap smear 1. Patient Preparation: The optimal time for collection of the Pap smear is two weeks after the first day of the menstrual period. The patient should be instructed not to use vaginal medications, spermicides or douches 48 hours prior to the collection of the Pap smear. The patient should also refrain from intercourse 24 hours prior to collection of the Pap smear. 2. Preparation of the Cervix: Lubricant jelly should be avoided as it often obscures cellular material making cytologic evaluation difficult, if not impossible. The speculum must be positioned so that the outer surface of the cervix appears at the end of the instrument, since a sample from this area is necessary for adequate specimen collection. The cervix should be free of excess blood, mucus, or inflammatory material. These may be gently removed with a dry gauze pad or a large-tipped swab without forcibly removing any cellular material. 3. | | 4. Submission: Label the specimen container with at least 2 patient identifiers (name and DOB preferred). Place completed cytology requisition, that includes appropriate clinical data, in the exterior pocket of the plastic bag. Send with the next available courier to the PSMG Cytology Department.

| | Conventional Pap Smear 1. Patient Preparation: The optimal time for collection of the pap smear is two weeks after the first day of the menstrual period. Specimen Collection: Equipment needed: a. Clean frosted-end glass slide b. Cytology fixative c. Cervical-Vaginal Scraper (spatula) d. Cytology Brush e. Cardboard slide folder f. Cytology requisition g. Plastic biohazard transport bag Collection: a. Using a pencil, write the patient’s full name on the frosted end of the slide. b. V: Take vaginal smear from posterior fornix with spatula end of cervical scraper. Spread material evenly on slide closest to frosted end. c. C: Take cervical smear by rotating the cervical scraper around ectocervix with special emphasis on the squamo-columnar junction. Spread cervical material evenly on the center of the slide. d. E: Take endocervical smear by gently inserting Cyto Brush into endocervical canal past squamo-columnar junction. Rotate brush one-half turn (180°). Gently remove brush without touching vaginal surfaces. Spread evenly on slide, farthest from frosted end, by rotating Cytology Brush (clockwise) to ensure deposition of all cells. e. Immediately fix preparations by spraying entire slide with cytology fixative. 4. Submission: Label frosted end of slide with the patient’s name (in pencil). Place slide, when dry, in slide container and then in specimen baggie with cytology requisition that includes appropriate clinical data. | | Ancillary Testing (CT/GC/HPV/HSV) FROM SUREPATH PAP SMEAR In addition to cytologic analysis, the following tests can be added to a SurePath specimen simply by marking the test on the PSMG requisition. Please include the appropriate ICD-9 codes. - Chlamydia trachomatis/Neisseria gonorrhea
- Human Papilloma Virus
- Herpes Simplex Virus types I and II
These tests may be added up to 21 days after the date of service without any additional procedure, or they can be requested directly on the PSMG requisition. HPV testing may be done as a conditional ("reflex") order based on the cytologic result of the PAP. Such orders may be arranged as a standing order, or on an individual basis. | | APTIMA® Unisex Swab Specimen Collection Endocervical swab specimens SPECIMEN COLLECTION AND HANDLING: 1. Remove excess mucus from the cervical os and surrounding mucosa using the cleaning swab (white shaft swab in the package with red printing). Discard this swab. NOTE: To remove excess mucus from the cervical os, a large-tipped swab (not provided) may be used. 2. Insert the specimen collection swab (blue shaft swab in the package with the green printing) into the endocervical canal. 3. Gently rotate the swab clockwise for 10 to 30 seconds in the endocervical canal to ensure adequate sampling. 4. Withdraw the swab carefully; avoid any contact with the vaginal mucosa. 5. Remove the cap from the swab specimen transport tube and immediately place the specimen collection swab into the transport tube. 6. Carefully break the swab shaft against the side of the tube at the scoreline and discard the top portion of the swab shaft; use care to avoid splashing of contents. 7. Re-cap the swab specimen transport tube tightly.
SPECIMEN TRANSPORT AND STORAGE: After collection, transport and store the swab in the swab specimen transport tube at 2◦C to 30◦C until tested. KIT STORAGE REQUIREMENTS: Store collection kit at room temperature (15◦C to 30◦C). | |

| | APTIMA® Urine Specimen Collection Urine Specimens: Male and Female SPECIMEN COLLECTION AND HANDLING: 1. The patient should not have urinated for at least 1 hour prior to specimen collection. 2. Direct patient to provide a first-catch urine (approximately 20 to 30 mL of the initial urine stream) into a urine collection cup free of any preservatives. Collection of larger volumes of urine may result in specimen dilution that may reduce test sensitivity. Female patients should not cleanse the labial area prior to providing the specimen. 3. Remove the cap and transfer 2 mL of urine into the urine specimen transport tube using the disposable pipette provided. The correct volume of urine has been added when the fluid level is between the black fill lines on the urine specimen transport tube label. 4. Re-cap the urine specimen transport tube tightly. This is now known as the processed urine specimen.
SPECIMEN TRANSPORT AND STORAGE: After collection, transport the processed urine specimens in the GEN-PROBE APTIMA urine specimen transport tube at 2◦C to 30◦C and store at 2◦C to 30◦C until tested.
